Low enzyme activity limits minoxidil conversion. Some topical additions may improve it. The evidence is modest but the interventions are low-risk enough to be worth knowing.
SULT1A1 activity is largely genetically determined, but enzyme expression in skin is not entirely fixed. Several interventions have been studied for their ability to increase sulfotransferase activity at the scalp, with varying degrees of evidence.
Tretinoin: the most studied enhancer
Tretinoin (all-trans retinoic acid) has a dual mechanism relevant to minoxidil response:
Penetration enhancement: Tretinoin modifies the stratum corneum, increasing permeability and allowing more minoxidil to reach the follicle. This is the more definitively documented effect and is independent of enzyme activity.
Sulfotransferase upregulation: Retinoids affect gene expression in skin cells, including potentially increasing SULT1A1 expression. The evidence for this specifically in human scalp follicles is suggestive rather than conclusive but is mechanistically coherent.
Clinical studies combining low-concentration tretinoin (0.025–0.05%) with minoxidil show enhanced outcomes compared to minoxidil alone. This is a well-established combination in dermatology. Compounded formulas combining minoxidil, tretinoin, and sometimes finasteride in a single vehicle are available through telehealth platforms.
The practical protocol for low converters
For users with confirmed or suspected low SULT1A1 activity:
- Add low-dose tretinoin to the minoxidil regimen (compounded combo or sequential application)

- Simultaneously explore oral minoxidil — systemic delivery may overcome some conversion limitations by providing higher follicle prodrug concentrations
The realistic ceiling

They cannot fully compensate for very low intrinsic enzyme activity. Can I add tretinoin to my minoxidil myself?
