Starting minoxidil right means a few specific things done before day one. This checklist covers all of them.

Starting minoxidil correctly means doing a few specific things before and on day one. Here is the complete checklist.

Before day one

Pre-start checklist

  • Confirm the diagnosis. AGA pattern (crown/temporal thinning, family history) or another minoxidil-appropriate indication confirmed by a clinician or consistent with established diagnosis criteria.
  • Choose your format.
    • OTC topical 5% foam: start today, no prescription needed, good adherence, faster drying
    • OTC topical 5% solution: start today, cheapest option (especially Kirkland bulk)
    • Prescription oral (0.625–2.5mg): requires telehealth intake, 3–7 days to receive, once-daily simplicity
  • Take baseline photographs. Natural light, day after washing and air-drying, no products. Overhead, front, both sides, crown close-up. Date them. These are your month-zero reference.
  • For oral minoxidil: Check baseline blood pressure and resting heart rate. Note any cardiovascular conditions or medications to disclose in the intake.
  • Note your baseline shedding. Approximate daily count from pillow, shower, brush. This is for comparison in weeks six to twelve when the initiation shed may begin.

Day one: first application

  • Shower and dry scalp completely — fifteen to twenty minutes air dry, or towel dry and wait five minutes
  • For foam: dispense half a capful onto fingertips, work gently into scalp across the affected area
  • For solution: apply 1mL via dropper in small amounts distributed across the thinning zone, directly to scalp not hair
  • Wait twenty to thirty minutes (foam) or thirty to forty-five minutes (solution) before styling or any hat
  • Wash hands after applying
  • Set a twice-daily reminder (morning and evening) for the next two weeks while the habit forms

Month one goals

  • Establish the twice-daily routine by the end of week two
  • Track any scalp symptoms (mild tingling is common and normal; significant burning or redness warrants contacting a clinician)
  • Take the month-one photograph set in identical conditions to baseline
  • Do not assess for hair results — none are expected yet

Know the timeline

  • Weeks 4–8: possible initiation shed. Expected and temporary. Do not stop.
  • Months 3–4: shed resolves, early vellus growth possible in photographs
  • Months 4–6: early visible improvement in responders
  • Month 12: first fair comprehensive assessment

The biology takes as long as it takes from here. Your job from day one is daily dosing and monthly documentation. That is the complete protocol.

Common questions

What do I need before starting minoxidil?

A confirmed or clinically suspected AGA diagnosis (or another hair loss indication your clinician is treating with minoxidil), your chosen format (OTC topical foam/solution or prescription oral), baseline photographs, and a routine plan for consistent application.

Should I get a prescription before starting?

For topical minoxidil, no — it is OTC and available today without a prescription. For oral minoxidil, yes — a telehealth platform intake or GP visit provides the prescription for the compounded low-dose formulation.

What should I monitor in the first month?

Baseline shedding rate (for comparison when the initiation shed begins), any scalp symptoms (irritation, unusual reactions), and dosing consistency. No hair results are expected in month one.
